Creation of custom fields is discussed in general in the knowledge base article entitled 'Configuring Custom Fields' (http://clockwisemd.desk.com/customer/portal/articles/2235801-configuring-custom-fields). What follows in this article discusses custom select dropdown fields specifically.
One of the most commonly-used custom fields in Clockwise.MD is the select dropdown field. Clinics can create a custom pick list from which patients or staff can select a single value. If the value desired does not appear in the pick list, the field can also be configured to have an 'Other' value which opens a free-text dialog into which the desired value can then be typed.
The pick list values can be entered in any order and re-ordered at a later point in time. It is extremely important, however, that commas NOT be part of the list value(s) as the list of values within the field is itself a comma-delimited list, and including commas in the values will cause the list to be separated at unwanted points.

If you originally entered a list of values and later wish to re-order the list, the list can be most easily re-ordered in the Team Admin > Custom Field module (Clockwise support technicians only). In the Custom Fields module, filter the list by the hospital/clinic ID, then find the field you wish to re-order and click Edit. The comma-separated list appears near the bottom. If you only have one or two values to re-order, you may be able to move the values in the horizontal dialog itself. If you have many values to re-order, it is best to copy and paste the entire list into Notepad (or something similar), make your edits, ensure that you have a single comma between all values except after the last value, and then copy-and-paste the list back into the field dialog, clicking Save at the bottom to preserve your changes.
Select dropdown fields can also be used to trigger the appearance of other fields by creating conditional fields that are linked to specific responses within the first select dropdown list.
A select dropdown question might be constructed which asks the patient 'Which of the following do you feel contributes most to your ill health?'. Pulldown choices might include:
HIGH BLOOD PRESSURE
RAPID HEART RATE
It is then possible to construct conditional follow-up text, number, and/or other select dropdown questions which could be triggered by the selection of one of the symptoms in the first questions, e.g., 'What is your weight (in pounds)?' or 'What is your blood pressure?'. Care should be given, however, not to make the questionnaire so long as to impede registration.
